Rhinoplasty for Short and Wide Nose?

I'm a 25 year old girl and feel that my nose is too wide and bulbous. I have a short nose and the part where my nose meets my forehead is very low. I want to make my nose narrower and higher(where it meets my forehead), maybe longer (Is it even possible?). Can all these concerns be addressed with a single rhinoplasty surgery? What procedures would it involve? Also whenever I smile, my nose takes on a very different shape with the middle part pointing downwards. Can this be corrected?

Well-performed Rhinoplasty Surgery may be employed to raise the height of your bridge and narrow your tip.

I read your concerns and reviewed your photos. In my practice, I use Implantech, ePTFE implants to add height to your nasal bridge. During your surgery, you could have tip refinement, and placement of a columella-strut which will support your tip and decrease drooping with smiling. Nasal lenghthening is difficult to achieve: and I don't think this is necessary for you. I hope this is helpful for you. Regards from NJ:

Thank you for your question. First, I would say that you side view is excellent and I would be very cautious about changing it. You have a nice straight nasal profile with good tip projection. Trying to "fill in" the area near your forehead could cause a deformity. A Tip Rhinoplasty can narrow your nasal tip but you will also need narrowing of the nasal bones to keep the front view in harmony. Yes, it can be done in one procedure

The pictures you presented are good and you concerns can be addressed with open rhinoplasty. For the bridge; elevating it with a diced cartilage graft will not only improve the shape but will also improve the length. For the tip; a tipplasty with cartilage suturing techniques will improve the width and the height. As for the nasal tip depression during smiling; a release of the muscle the pulls the tip downward will correct this problem.  I hope this is helpful.

The situation you present is not an unusual one. Your concerns can essentially be grouped into the following: LENGTH: Making a nose longer can be accomplished with grafts. Depending on the nose, you either add length by derotating the tip, augmenting the bridge (as you showed) or adding a 'precrural' graft to the columella. Hello, Yes, it is possible for all of your issues to be corrected in one surgery. Augmenting the dorsum can help improve the angle at which your nose meets your forehead. This can be achieved using your own cartilage. The nose will appear longer following this procedure. Your tip may be brought up as well, which will result in less of a droop when you smile. Rhinoplasty, Nasal Surgery, revision Rhinoplasty.

Yes, you could build up the bridge of your nose, thin the nasal bones and tip cartilages.  The tip appears, from your photos, to be rotated at approximately 110 degrees from the upper lip which is the proper aesthetic angle for a female nasal tip...so I don't believe that it needs to be lengthened (another way to say the nasal tip is rotated downward at 100 degrees or less).  All of this could be done with a single closed Rhinoplasty.  An open Rhinoplasty IMHO would... more

I agree with your nasal evaluation. You can augment your nose at the naso-frontal angle with cartilage or fascia - it is unlikely you will need to lengthen your nose after increasing projection as we just described.   The entire nose can be narrowed and refined. A small muscle  (the depressor septi), which pulls the tip down when you're smiling, can be interrupted. All of this can and should be accomplished with your rhinoplasty procedure.

HI. 1) You actually made a good analysis of your nose and you provide good pictures. Fortunately your anatomy is favorable and you can expect the improvement you want without the need for an open (more invasive )rhinoplasty. 2) The worst thing for you would be to get a "standard" rhinoplasty, where the "bump" is removed. Certainly your concerns can be addressed during rhinoplasty surgery. If you feel you dorsal height is a bit low there are several different things that can be used to raise this area up and this will also give the illusion that your nose is narrower.  Tip narrowing and nasal width narrowing can be achieved as well.

A rhinoplasty will correct your wide nose and the rest of it, too. Your surgeon will be able to make your nose narrower, to fill out the upper profile indentation, to lift the tip when smiling, and to make your nose fit into your face. All of your desired changes in your nose could be achieved in a single rhinoplasty procedure. The most controversial part of your procedure would be how the upper part of your nose (radix) will be made higher and built up closer to your forehead. Some plastic surgeons may choose a synthetic material, others use cartilage. Each has its own advantages and disadvantages. Otherwise, changing the tip shape and its projection and eliminating the pulldown on the tip are standard rhinoplasty changes. more

A: Rhinoplasty for droopy tip and wide nose

I agree with raising the nasofrontal angle to increse the length and give a thinner appearance on the feontal view. By clipping one of the muscles on columella the droopy tip will improve when you smile. It appears that you have thick skin which would limit the tip definition, even if a tip graft is used, but I cannot tell for certain from the photos. Lastly, the side walls on your tip appear to be weak, too much cartilage removal may result in a pinched look tip. Dr Siegel more

Rhinoplasty for Bulbous Tip and Increase Bulbouse Tip

Hi, I typically use a custom carved silastic implant to raise the height of the bridge and elevate the naso-frontal angle (the height where your nose meets your forhead). This will also make your nose appear longer. To refine the tip of your nose, a cartilage tip graft and defatting the tip will take away the bulbosity.
