I had a rhinoplasty over 5 years ago. I am now left with an unsightly scar on my columella. I have tried lasers, but that was unsuccessful. My question is during a secondary surgery could the scar on the columella be placed at the base where a lip lift scar is? I think that if the scar was placed in the crease it would be less noticeable.

Bad columellar scar after rhinoplasty.
The original scar should be revised, not the one at the base of the nose. In 35 years of making this incision, I have never had to revise one, so it should heal fine.

Rhinoplasty Scars can be revised
I think that revising the current scar should be the best plan for you. The columnella incision should be closed meticulously and heal very well. If for some reason it hasnt healed just right, i would advise a scar revision.
Closing the insicion with fine sutures yields beautiful results.

Rhinoplasty and columellar scar
If the scar on your columella is in the usual incision site for an open rhinoplasty, it probably will not be possible to relocate the scar to the base of the lip. Depending upon its appearance, you may be a better candidate for a meticulous scar revision without changing the general location of the scar.

Rhinoplasty Scar after Rhinoplasty
There were 4 of us who first did open rhinoplasties in the United States over 35 years ago. The columellar scar has never been a problem - I strongly suggest you have a minor scar revision without moving the incision.

Prvevious open rhino and relocation of undesireable scar
I would discourage you from this. The skin between the two incisions would have a higher likelihood of poor heaing and could potentially create a more difficult situation.

Revising an Unsatisfactory Columella Scar
Columellar incisions are used so frequently in rhinoplasty because they tend to heal very well and are often almost imperceptible. However, if you have an unsatisfactory columella scar, I would suggest that the best option is to revise the scar. This can usually be done in the office and with very careful, meticulous technique, a good outcome should be achieved. Columella scar revision is typically an office procedure with local anesthetic.
The vast majority of trans-columella incisions heal inconspicuously. In my practice, I've seen a handful of rhinoplasty scars that are obvious since one of the corners isn't alligned perfectly. When this is recognized, a scar revision may be performed with minimal inconvenience.
