Should I Consider Revision Rhinoplasty?

I am a 22 year old female and in December 2008, I had a Rhinoplasty and a Septoplasty. I went back to see my doctor a week ago because I have a hard bump at the tip of my nose that hasn't gone away. He injected some type of steroid but I haven't noticed a difference.

I was advised that it may take some time to see if the steroid injection will actually work, I was told I should wait at least three months. He told me this bump is common and may just be extra cartilage and if I really want I can consider having surgery again to correct it. I would be okay with surgery because right now my nose doesn't look even. However, I am worried that this may have been caused because he may not be a good surgeon and I don't want to come out looking like Michael Jackson.

Please give me advice. I don't know what to do. Should I wait considering it's been only about a year and a half? Thank you!

Generally, one year is an appropriate time to wait prior to considering revison rhinoplasty. In your situation, one would probably want to wait an additional few months after steroid injection to see what effect this has. Experienced rhinoplasty surgeons may need to do a touch up here and there.  In experienced hands, the revision rate should be in the less than 5% range.   The first thing to consider with revision surgery are...what are your goals?  What do you want? Need for rhinoplasty revision is not uncommon even for experienced surgeons

The fear of having a "Michael Jackson" nose has become a standard among patients in most facial plastic surgery practices. This is obviously not the result that any of us desire. If you trust your surgeon and they are experienced with nasal surgery, it sounds like a revision surgery might be in order. Rhinoplasty is the most challenging surgery that is performed by Facial Plastic Surgeons. Revisions are not uncommon even among the most experienced surgeons.

Small Surgical Revision May Be Considered

It sounds like you have a bit of extra cartilage at what we call the “supratip” region of the nose, and this can be due to either a bit of cartilage was not removed, and/or the nasal tip with healing settled back and reduced tip projection, exposing the cartilage. Revision rhinoplasty can occur in the best of hands

Most surgeons like to wait one year for a revision procedure.  The best person to do a revision is often the initial surgeon since he knows exactly what was done and may have a good idea how to address your concern.  I suggest you outline your concerns to the surgeon and see if the surgeon can address your concerns.

Revision rhinoplasty timing

There are several things to consider. Do you feel your surgeon will be able to help you? If its a small imperfection, then probably. Do you need a second opinion? The soft tissue envelope of the nose has the potential for great changes over many years, but the majority of that change is certainly observed in year one. If there is a unsatisfactory sequela from your initial procedure at a year, it is unlikely to get better without an intervention. It is critical to seek the advice of a rhinoplasty surgeon with experience in structural grafting when that time comes so that both the airway and appearance can be improved.

It is wise to be concerned about revision surgery. Many times revision surgery will help and at times they may make the problem worse. Waiting is not bad idea but after 2 years not much changes.

The reason for this is that it can take 12 months (or longer) for the majority of the post-operative swelling to subside and for scar tissue to mature. As you can see from these answers, timing is critical in considering revision rhinoplasty. Ther farther out you are from your original surgery the better. Only under rather exceptional circumstances would most surgeons consider re-operating on you in less than 1 to 2 years after the first operation. Might need a revision Rhinoplasty

Revision rhinoplasty if needed should be done after 12 month from the first procedure to allow complete resolution of the swelling. It seems like your bump could be fixed with a simpler revision. Either continue to follow with your doc, or find a board certified plastic surgeon for a second opinion. Good Luck!

Hi Surfergirl, Consult with your surgeon. If you lack confidence in the surgeon's abilities then seek other opinions. Many excellent rhinoplasty surgeons now offer complimentary consultations. Revision Rhinoplasty is reasonable after a year

If your surgery was December 2008 then you should wait to have any revision surgery on your nose. It will take about one year for the tip to be completely healed. If your surgery was "about a year and half" then you should consider having a revision because the irregular cartilage bump will probably not go away.

See a revision rhinoplasty specialist

I'm confused. You said Dec. 08 and then you said one and a half years. If it has been more than a year, the result will probably not change. Therefore, if your tip doesn't look satisfactory to you, you should see a rhinoplasty revision expert and you should get a good result.

You may want to seek out second opinion

Dear Surfergirl, It sounds like surgery was done just a few months ago (dec '08), so it is usually too early to make any decisions regarding revision at this point. Steroid injections in the supratip can often help if there is residual swelling or small amount of scar tissue forming. However, what you are describing sounds a bit different- it's possible that it is scar tissue but if it is hard it usually turns out to be the top of your septal cartilage or part of the tip cartilage.
