What Needs to be Done for Hole in the Teeth?

My husband has hole in the outer layer of the teeth, Usually if a tooth is decayed, the hole appears on the top! I just need to know the reasons for the outer layer side hole. How this can be treated, does he need just a filling or a crown. Does the teeth can be saved? Does smoking has adverse effects on teeth, cuz my husband smokes daily. I suspect the reason for hole could be due to smoking.

Hole is caused by Bacteria

If your husband truly has a "hole" or "cavity" aka dental caries, it is caused by a bacterial infection. Enamel is weakened over time by an acidic environment and biofilm which evenutally causes a weak spot that bacteria can begin to grown in. It is treated by removing the dental caries and creating a "cavity" which is then filled with some artificial material.
