Do you still drop and fluff like the first time with a bilateral capsulorrhaphy? I had several sutures put in and initially was 420cc mod, I am now 425cc o/f to 475cc. What is the best bra for the 24/7 bra wearing after PO? My PS says I need to keep them supported 24/7 with an underwire until I see him next.

No, in my experience, there is much less “dropping” of breast implants seen after capsulorraphy surgery, compared to initial breast augmentation surgery. In regards to the use of bras, I would suggest that you follow your plastic surgeons recommendations; he/she knows your situation best that is ultimately responsible for your care.

Answer: Pocket Revision/internal Bra Drop and Fluff?
Since implants already have been in place, and the pec muscles have accommodated, the early high riding implant phenomenon is less common in a revision operation unless the new implant is much larger than the first. I would defer to your surgeon's recommendation for a bra. It is important to provide support until the capsulorrhaphy areas have healed.

Answer: Implant "dropping" and a capsulorrhaphy.
The phenomenon of implant "drop" is more about the breast tissues relaxing and allowing the implant to fill out the lower pole of the breast. With a slight increase in implant volume (420->475), this will not put too much additional stretch on the tissues. Also, it is obvious that the implant pocket is too large, hence the capsulorrhaphy which further reduces the likelihood of seeing an implant "drop." As for bras, supportive cup bras work very well. Underwires allow for accurate positioning of the bra. I personally do not like the sports bras as they compress rather than support. If you have difficulty getting fitted, try going to a mastectomy department at a department store (Nordstrom's for instance) where they will be happy to size you appropriately.

I doubt you will see as much the second time around because you had a capsule tightening procedure and you went with bigge implants.

Answer: Correct bra to wear following a capsulorrhaphy
Every plastic surgeon has their own post operative protocol following breast surgery. If he recommends an underwire bra then this would help to support then newly created inframammary crease and keep the weight of the implant off of the capsule repair.
