Panniculectomy, Weight Loss Surgery or What Would be Best for Me? (photo)

I am in my early 20s. Beginning of last year i weighed 500 plus pounds i started dieting in middle of last year up until now. I have only lost 30 pounds. I get constant rashes and weeping from under my stomach ( the pic shows how far the gut extends it goes right below my pubic area) and behind my knees i want to exercise but I am in constant pain. My insurance does not cover weight loss surgery I have no credit. I just want the pan nus removed so I can exercise. What should I do?
