Hi. I had double jaw surgery 3 months ago to fix prognatism.I also had malar implants. So far I like the results but there’s something that’s bothering me. I never paid many attention to my jaw angle, because pragmatism gave me a “manly look”. I would like to know if jaw implants can be good to improve my look by “pushing” down my jaw angle. Besides I have a big asymmetry that I can notice more now after surgery. (Sorry I don’t have a photo). And sorry for the beard, it help me to “hide” may jaw.
Answer: JAW IMPLANTS AFTER JAW SURGERY Jaw implants and bone grafts can be used to adjust the angle of the mandible to correct asymmetry and improve the jaw line after orthognathic surgery. However without a physical exam it is difficult to say definitively if augmenting the jaw will correct the specific deficiency that you are unhappy with. Before considering any type of additional procedures, I recommend giving yourself time to adjust to your new look because your issues and concerns could resolve with time. For example, some of your asymmetry could be a result of swelling that is still lingering after your surgery and three months may not be enough time to resolve it all. Some swelling and excess fluid can remain for six months to a year after surgery so I would recommend waiting at least six months before considering any additional surgeries. Mandibular angle implants can be used to help define and provide good projection of the angle of the mandible that may have been lost during your orthognathic procedure to correct your prognathism. That being said, mandibular angle implants do have a higher risk of infection that malar implants and chin implants. I would definitely allow at least 9 months of healing after your orthognathic surgery prior to proceeding with angle implants. Loss of the angle definition can happen depending on the type of lower jaw surgery you had. If you had intraoral rams osteotomy (IVRO) there is a higher risk of losing that definition than if you were to have bilateral sagittal split osteotomies. I would also suggest having custom mandibular angle implants fabricated. This can be done by getting a new CT scan of your jaw, then sending it to an implant company such as Implantech. The surgeon and technician can fabricate the implants to provide the best possible outcome and symmetry.

Answer: Angle implants Hi. I would wait for one year and see if you still care about your gonial angles at that point. I think you are still getting used to your new look. If you still care after one year then gonial implants are easy to place and could be right for you.Yours,Julian Hirst
