I plan on having a 3rd revision rhinoplasty to remove an implant & excess bulky scar tissue in the tip. A surgeon suggested alar base reductions to narrow a bulbous, heavy tip. What are the drawbacks for this in thick skin? I have rounded features & am worried about anything that will lead to what I hope to undo w/surgery 3--a bulky, unnaturally overprojected tip. What are the dangers of alar base reduction, other than keloid scarring? Can is lead to nasal collapse or pinching?

Thank you for your question. My concern about alar base resection is that first it could restrict your breathing. Secondly there will be scars in the alar crease which you must be aware of. I agree that some tip elevation may help the problem as at least on the frontal...
The best course of action is to allow the healing process to be completed. At that point you can truly asses the nose and whether or not any further surgery is needed.
You reduced the nostrils, which is possible with an alar base reduction. You also reduced the bulbous nature of the tip, which can only be improved with a tip plasty. This can be done in closed fashion at the same time as the alar base reduction.Kenneth Hughes, MDLos Angeles, CA
