Nail Psoriasis - or Something Else?

I have had this nail condition on and off for about 12-15 years now. The only symptom is onycholisis (separation of nail plate) which makes the nails look white, yellowish. I have seen a couple of dermatologists over the years and their analysis has been similar - it's not fungus and most likely it is psoriasis. 1) Is there a definitive test to confirm nail psoriasis? 2) What is the most efficient cream/ointment for nail psoriasis? 3) Are there any really good derma/clinics anywhere in US?
