Can my Scar Be Improved? (2 Abdominal Surgeries from Bowel Disease. First Surgery 2008, Second 6/2012) (photo)

I had an emergency colectomy 5 years ago, which left me with a long vertical scar with suture / track marks along the sides. Six months ago, I had another abdominal surgery, where the surgeon went through the bottom half of the old scar, and stitched up a previous colostomy site. I would ideally like the scar to be shortened, made less noticeable, and revise the indented colostomy site and remove the track marks. What do you think would work best in my case, and what outcome should I expect?
