Multiple procedures

William Bruno, MD answers: Multiple procedures for a healthy 25-year-old?

Hello, I am 25 years old, 5'2'' tall and I weigh 135 lbs. I have not had a baby yet but I want to have a small tummy tuck and liposuction done all over my abdomen, back, love handles, inner thighs, under my chin, and take the good fat and do a Brazilian butt lift. Can I have that all done at the same time? I am also in very good health. I would be thankful if you reply. Thank You!

Based on your height and weight, you may be a candidate for liposuction and a fat transfer to your buttock, depending on the quality of your skin. If you are planning to have children in the future, you may want to consider delaying your tummy tuck until you have completed having children. If your lower abdominal area is free of stretch marks and loose skin, then perhaps liposuction alone is the answer. Again, you would need to be examined by a board certified plastic surgeon prior to any planned body contouring/liposuction procedures.

Provided the amount of fat removed is less than five liters, which I imagine it would be in your case, then these procedures can be combined safely at the same time. Be careful to avoid sitting for about two weeks to maintain the lifespan of your fat transfer to the buttock area.

Your surgeon may want to make arrangements for you to stay overnight at an after care facility to be observed overnight.

There is no exact formula for determine the maximum number of procedures. Your surgeon should gauge the accumulated risks to all the procedures. This is especially true for liposuction where the risks increase with the total volume removed but also, perhaps more importantly, with the total surface area of the body that is traumatized. When you mention liposuction "all over" mulitple areas, it suggests that you are perhaps not an ideal candidate for the procedure. Liposuction is best for localized fat and certainly not for weight reduction. Someone who is chunky all over or moderately obese all over risks poor results and complications from over-suctioning. The more you can limit the areas needing contouring, the better candidate you are for liposuction. Proceed carefully.

It may be possible and safe to do the procedures you describe, in a select group of patients for the right reasons.  If you have not yet had a consultation with a board certified plastic surgeon, a member of the American Society of Aesthetic Plastic Surgery(ASAPS) or the American Society of Plastic Surgery(ASPS)--that is the first place to start the decision making process.

You mention "small tummy tuck" and that you may have a baby in the future.  In general i would recommend waiting till your family is complete and finished with pregnancies prior to consideration of a tummy tuck.

Regarding the multiple areas of liposuction, your listed height and weight would suggest that the volume of lipo aspirate would be reasonable. The risks of liposuction seem to increase when more than 5 liters are removed in one setting. If more than 5 liters is removed at a time you should spend the first night after surgery in a monitored setting.

A: Actual multiple procedures safety is impossible to pin point

Dear Sexy,

In reading your description of your body, body weight, and what you are looking to accomplish, I am wondering if liposuction will ultimately accomplish what you are hoping for. In particular, as a young woman with no major body scars, I am concerned that you will feel with a scar on your lower abdomen stretching from one hip to the other. No matter how the plastic surgeon may tell you that it will be hidden or it heals beautifully, these scars are ugly and you may very much feel that your body was violated in a way that is difficult to psychologically deal with.  It is probably that this horrendous scar will make you feel anything but sexy.

Women who have had a c-section already have a scar and often a big pooch of fat that the obstetrician created by pushing up a bunch of fat in the tummy to make a smooth place to make a full thickness incision to get to the baby.  Pregnancy may have also over stretched the abdominal wall muscles creating a weakness that can be corrected with the open tummy tuck. Here, the trade offs begin to make sense. For you, I would strongly counsel you to reconsider.

The surgery you are considering will not transform you. You need to consult with a dietitian to develop a long term eating plan that makes sense for your small body. This combined with an exercise program will help get your body to be truly sexy for the long term. My best advise is you are much better off taking the money you are considering spending on surgery and invest it in personal fitness training. Ultimately, what do you think will make you happier?
