The most effective medication for blackheads is tretinoin, adapalene or tazarotene cream.  They are only available as a prescription from your physician.  Over the counter medication includes salicylic acid in a wash or wipe form and benzoyl peroxide gel.  You should be assessed by a dermatologist to see which treatments would be best for you.
