The cost of a mastectomy is determined by the surgeon operating.  Everyone is different.  Insurance should cover it.

Mastectomy covered by insurance

Mastectomy is a procedure that is covered by insurance 100% of the time. You did not mention the indication for mastectomy but it does not sound like a bad diagnosis of breast disease. Your coverage will depend largely on the diagnosis.
