Not much

Male breast reduction through the armpit is basically liposuction. It will not affect the nipple complex much, however, it may shrink some. For true gynecomastia you need to have gland excision. see video

Male breast reduction through the armpit seldom is indicated.

Excision of male breast tissue (gynecomastia) is required about 90% of the time. Liposuction, which can be done through the armpit, is indicated in less than 10% patients because breast tissue is only partially fat. The glandular portion cannot be removed my liposuction so excision is required.

Male breast reduction through the armpit

If you have a reduction of the chest through the armpit, it is likley that it is just liposuction using a fairly concealed incision. While there can be some numbness of the nipple initially after srugery due to swelling, it is unlikley that it owuld be permanent.
