Late occurring seroma/hematoma/contracture are most likely causes. Best to follow up with your treating surgeon.

You may need ultrasound of the breast

Your breast swelling and pain can be due to seroma, infection or contracture. Please see you plastic surgeon and you may benefit from ultrasound for further evaluation.

Lumps cold be result of wrinkling of the implant envelop

Since your implants were placed above the muscle, it is possible that wrinkles or creases in the shell of the implant could be more easily felt than if they were placed submuscularly. I would recommend that you consult either the plastic surgeon who did your breast augmentation or another plastic surgeon to see what is the cause. You should not be experiencing a drastic change in the size of your breasts.

Go see your doctor

It's difficult to give advice without examining you to determine what is going on. I would suggest that you call your surgeon and go in to be examined. This is the only way to know what is happening. Best Wishes!
