There are several potential causes for this wound after earlobe reconstruction.  It may be purely mechanical, related to massage of an immature scar, or potentially from extrusion of a resorbable suture.  When dissolving sutures are placed superficially, they may spit out through the incision a few weeks or months following surgery prior to resorption.  Infection should also be ruled out.  I recommend applying Vaseline twice daily to promote moist wound healing, avoidance of scar massage until well-healed, and scheduling follow up with your plastic surgeon for a clinical examination.Best, Stephanie Power MD, MSc, FRCSC

Thanks for posting your question and photo. It is unusual to have a scab falling off at 3-4 months after any surgery.  At this time post operative you should be very well healed.  My feeling is that there may be a suture that has prevented the area from healing.  Sutures may be absorbable or not.  Its best to get re evaluated by your surgeon to make sure your healing becomes complete.

Please returned to your original physician to make sure you do not have an active infection that requires treatment.
