I have seen abdominoplasty patients who either have what appears to be a very large pubis & or fatty mons. Is there a surgical solution to these outcomes?

Liposuction of the mons pubis during abdominoplasty
Liposuction of the mons pubis can and should be done with the abdominoplasty.
If the mons pubis is loose, there are specific steps that can be taken to tighten it, wiith sutures and resection at the top of the mons. It is important not to overelevate the mons or pubic structures can become unusually visible.

Big puffy mons and flat tummy
This commonly occurs and can be prevented or treated depeding on the underlying cause. Commonly after massive weight loss a surgical reduction and lift of the mons is required. After most pregnancies, simple repair of the SFS will minimize the tendency for this appearance. However, in some patients either direct excision or liposuction is a useful approach to manage the truly full mons.

Mons pubis in relation to abdominoplasty
The mons pubis should be taken into account when an abdominoplasty is performed. Often times, contouring with fat removal is all that is needed, but sometimes excess skin needs to be removed too. The surgeon should talk with each patient and take their individual anatomy into account.

Mons Pubis Liposuction with Tummy Tuck
I have to agree with you on this on. I think the mons is overlooked during many tummy tucks. Many patients could do with some liposuction in this region. However, others will have a more pronounced pubic bone compared to before surgery.
It needs to be a "game time decision" whether it should be liposuctioned or not. But some evaluations needs to be done.

Surgical options for excess fat or skin of mons pubis
Many patients who request abdominoplasty also have excess fat and or skin of the mons pubis.
If the patient has only excess fat and good skin tone, I perform liposuction of the mons area with the tummy tuck procedure.
If the patient has excessive skin and a publc lift is required, I stage the surgeries. My concern is that the pulling down force of the tummy tuck and the pulling up force of the pubic lift will place too much tension on the central part of the incision, leading to wound...

There are solutions for a large or fatty mons pubis
Yes, liposuction of the Mons is a reasonable thing to do if the fat pad in that area is prominent. I do this quite frequently at the time of the abdominoplasty. I have also had patients that have a drooping Mons that abdominoplasty corrects. And, still others that their Mons is so stretched out from being overweight that I do a Mons reduction surgery. But, I agree, no one likes to have an excessively large mons.

Liposuction, judiciously applied and carefully executed, is an excellent adjunct in any abdominoplasty procedure.
I often do perform a bit of liposuction on the mons if the patient's anatomy indicates the outcome will benefit.

Liposuction of the mons pubis is often performed in conjunction with abdominoplasty surgery to better contour the pubic area. Sometimes the fat within the mons is removed surgically as well. Often, abdominoplasty patients have excess fat in their mons and to leave it in detracts from the results. Many plastic surgeons include this with the price of the surgery.

The mons and other areas adjacent to the abdomen are routinely contoured with liposuction as part of an abdominoplasty when needed. You should of course point out your concerns to your plastic surgeon and be clear on what will be included.

Tummy tuck is a body contouring procedure and the whole of the abdomen should be taken into account to realise the best results. Mons pubis and abdominoplasty
The mons can swell after an abdominoplasty or can look enlarged relative to a flatter and tighter suprapubic area. A surgeon can certainly reduce the skin and liposuction the fat of the mons in conjunction with an abdominoplasty.

Yes, depending on the patient's anatomy, I often perform liposuction of the flanks and the mons pubis at the same time as the abdominoplasty. There are many new ways of contouring the mons pubis region. Certainly liposuciton in helathy patients with fatty tissue excess will help reduce the fat, however in many there is also an excess skin that hangs. This hanging skin can create issues with hygiene and can be uncomfortable and visible through clothes.

Liposuction of the Mons pubis WITH a Tummy Tuck
With child birth, obesity and aging the Mons pubis sags, enlarges and rotates inferiorly.
While we want to elevate a sagging Mons to a youthful position, we do not want it TOO high, nor too pronounced in height.
