I have blurry vision in reader eye 5 weeks after blade lasik. Any suggestions?

Sounds like you had a displaced flap and macro-striae.  Minor folds that do not affect vision are generally called micro-striae.  Treatment is necessary for macro-striae.  
 In every patient, prior to flap lift we use a corneal marker to insure that flap is aligned after the laser treatment.  Some surgeons, judge the position solely by the width of the gutter (space between the flap and remaining cornea).  We float the flap back in position noting the alignment marks and wash out the interface debris with a stream of salt water (BSS).  After returning the flap to the correct position, we use a special sponge to smooth the flap and use a pump and tubing to blow a fine stream of air to seal the flap in position.   Some use a clear plastic device to press the flap in position.  The device or sponges if applied improperly can result in folds in the flap. If the flap is not allowed enough time to set in place, it can become dislodged with removal of the speculum,  blinking or gentle trauma especially in the early days after surgery.  We place protective shields and encourage our patients to sleep for the first week with a shield to avoid dislocation of the flap and wrinkles in the flap. 
I see that your procedure was done a couple of years ago.  I would think after the flap was re-positioned your vision improved.  

The fact that you require a short working distance might indicate an undercorrection in your reading eye. The flap striae may also be at issue and this is an issue that bladeless procedures have eliminated. At five weeks it is too early to consider an enhancement but this may be necessary eventually.
