Im 35 yrs old and have 36ddd breast. They are starting to sag and would like to stay the same size but get back to a fuller, perkier breast. I have been blessed enough that they dont point straight to the ground. Is there a way of doing it without a breast reduction? Afraid of the scar down the center of the breast. Can I put in a bit of a larger implant to fill them out or do I have to do both? What would be my options? thanks

Breast lift

Looking at your photo it would appear that a breast lift would be the best option.  It would be a natural way to have the fullness that you desire while keeping in mind your frame.

36DDD and wants implants / avoiding a lift

Your question is a common one.  While a breast lift can be done without reducing your size, the trade-off of incisional scars is sometimes hard for patients to get their head around. Adding an implant to your natural 36DDD (=36F) size is not the right answer.  Why?: 1)  It would add a cup or two to your size...and you said you wanted to remain the same size 2)  Implants do not lift the breast....you'd just have a bigger version of what you have now. 3) more

Breast Implants or Breast Lift

In your situation, the best answer truly is a breast lift. That will restore your upper volume (the 'perkiness'). Adding an implant would only add to the weight and increase the droop. Unfortunately, sometimes you have to accept the scar to get the shape you want.

Breast lift versus implants for sagging...

Breast Lift

You could have a breast lift to move the nipple and areola (dark part around your nipple) back up to the middle of your breast and remove excess skin.  This would involve scars on the front of your breast but they tend to fade well over time.  A breast augmentation would not accomplish this goal - an implant may fill out your skin better but will not move your nipple back up to the middle of your breast.  Good luck!

I agree with my colleagues that an implant alone is probably not going to give you the look that you want. A lift and a small implant often gives the best result for prolonged upper pole fullness and the perky look. You will have to weigh the pros and cons and make a personal decision here.

Breast implant or breast lift

I agree with the group in that adding an implant will not solve your complaints.  You may have a temorary fullness but will certainly be larger and will eventually have more breast sagging.  You would be best served with a breast lift. If you are concerned with scarring find a local surgeon that performs a vertical lift as these types have significantly shorter scars.

Large breasts with ptosis implant, lift?

You have large breasts that have significant ptosis (sagging).  Regarding your question, you can not add an implant for fullness AND stay the same size. The scars from a breastlift fade and are usually not bad after healing. Adding an implant may cause increased sagging, or if placed where it should be aesthetically, will look "funny", as the breast will tend to "flag" off of it. So a lot to think about.   sek

Vertical breast lift will benefit larger breasts

In my opinion, implants would not provide you with sufficent lift and could result  in the appearance of a larger more "saggy" breast. You have to weigh it out and figure out which is worse, a well maintained scar or saggy breasts after augmentation.

Based on your picture, a breast lift may be better. Implants will make your breasts larger and over time the may sag more.

Breast Lifts Create Perkier Breasts, Breast LIfts Do Not

One of the common misconceptions is that breast implants will have a significant breast lifting effect. This is an erroneous concept. Placing breast implants in your breasts will only make them bigger but not necessarily any perkier. Only a breast lift with the resultant scars can achieve what you want...but at the expense of the scars. Implants don't lift breasts

Unfortunately at your size, implants won't help and you can't avoid the vertical scar either.  You ned a true breast lift and one done by an excellent palstic surgeon to get a great shape.  Then the scars will be worth it.

Implants in large breasts?

You do not have a volume problem.Implants will only add weight and made you even saggier. Only you can decide if you wanted lifted breasts with scars, or saggier ones without them. In my opinion if you go with a lift you should consider having some breast tissue removed to better maintain the lift.

Sagging Breasts Not Corrected by Implants

If you currently are wearing a 36DDD bra and are sagging, there probably is very little chance of correcting this without some sort of breast lift.  This is especially true if your nipples are at or below your inframammary crease.  I understand that you want minimal scars, but you always must weigh the benefits from surgery with the disadvantages and risks.

Larger breasts need to be perkier

If you feel that you have enough breast tissue then depending upon how the breasts look, you may need a small lift. Unless you want to be bigger, you do not need an implant.
