Nasal Reduction Surgery: Can Incision be Made on Natural Nose Crease?

The technique mentioned by my surgeon is to cut my nostrils in front of the natural crease where the nostril meets the cheek. Is there a procedure were the nostil could be cut directly on the crease so that it looks more natural?

Also, I have annoying lines where my nostril and cheek meet (maybe smile lines, they're always there smiling or not. Would it be possible to bring in these lines during the procedure for a more smooth appearance afterwards? If not, what would you recommend for this?

Nasal reduction and where incisions are made

It is best to have the incision for alar reduction placed inside the nostril rather than around the alar facial crease.  That will be detectable and noticeable and many patients are unhappy about that incision placement.  Any smile lines would be best treated with dermal filler as a temporary measure.

Incisions on the nostrils

It sounds like your surgeon is suggesting a alar resection (reducing the nostrils and/or narrowing the base of the nose).  This technique does place the final scar in the natural crease.  Clarify with your surgeon, but I am sure he will confirm this.  As for the other lines, it is difficult for me to give you an opinion without a photo.  If they are precisely where you describe them to be, then there is a chance that they may be improved, but now I am really just guessing. more
