Are Incisions in Wrinkles in the Forehead Normal in Browlifts? (photo)

I am a 70 yo woman, getting a blepharoplasty and an eyebrow lift. I have a receding, thinning hairline and the surgeon plans to make the incisions in "wrinkles" in my forhead. However, my forehead is smooth, showing slight wrinkling only when I raise my brows. I am wondering if this is a usual procedure. Will it leave any lumps or bumps in my forehead or will my forhead be smooth as it is now? Will he cut muscle or only skin? Will the weight cause the scar to stretch?

Incision in forehead wrinkle for brow lift

We do not recommend the forehead lift incision in the middle of a wrinkle across the forehead. Placing the incision in the wrinkle will only deepen the wrinkle and leave a scar going across the midportion of the forehead. For a receding hairline we place the incision at the hairline. The question you’re asking about your procedure where an incision is made inside a wrinkle in the forehead is in my opinion should be reconsidered. The procedure your surgeon is recommending is called a mid forehead lift. This procedure is generally more advisable for males with thicker skin and deeper lines. Forehead lift scars are unnecessary

You submitted a very nice photo showing that you indeed have heavy upper lids and a low brow complex. Making incisions in your forehead is certainly one way to lift your brow, but usually not performed in someone your age who is looking for a cosmetic and functional improvement. An endoscopic browlift done at the same time as upper eyelid surgery would give you a very nice result. Cosmetic vs Functional

Browlifting can be done for cosmetic reasons and sometimes for functional reasons to help patients see better. Most surgeons would not recommend midbrow or eyebrow incisions for cosmetic indications, unless there are extenuating circumstances. Looking at your photo, the degree of brow droop does not show compromise of your visual field, so you would have a much better result with a hairline or endoscopic approach. Although a brow lift through the "forehead wrinkles" works, I personally think that it should be avoided in anyone concerned with their appearance, especially a woman. Furthermore, you are describing that you have a smooth forehead, and this procedure will probably make your forehead crease thicker and more noticeable. There is always another solution. Numerous approaches and incisions are available for browlift surgery

I would agree with my colleagues that based on the limited photo you've provided that it does appear that you are a good candidate for upper eyelid and brow surgery. How exactly to perform the browlift depends upon your hairline and the appearance of lines in your forehead. For someone with severe brow drooping I will typically recommend the tricophytic approach to someone with a high forehead or a coronal approach for the low to normal forehead. Irregular trichophytic forehead lift is best.

We developed the irregular Trichophytic forehead lift 30 years ago which we described in our text book. The incision is made by burying the roots of your hair at the hairline. The hair grows thru and in front of the scar. In patients with a high hairline the hairline can be lowered at the same time.

Blepharoplasty and browlift

Your posted photo shows significant brow ptosis so browlifting is in order. I have done this by removing skin between adjacent forehead wrinkles but only in patients with deep forehead wrinkles and these tend to be men. Tapering at the outer edges is a bit tricky and has to be balanced with a tapered removal of temple scalp.
