I'm planning to get a BBL out of state. What documents do I need to bring besides my medical records and I.D for pre-op?

going out of state to get a bbl what are all the documents i need to bring besides medical records and i.d? please tell me any and all things I should bring.. since I'm out of state, i don't want to forget anything vital and not be able to get my sx. thank you doctors!
