Effects of pregnancy on breast reduction
The effects of pregnancy on the appearance of your breasts can vary with surgery and they can vary without surgery. Breasts can become larger, smaller and/or more sagging (ptotic). It is very difficult to predict .
How will pregnancy affect your breasts?
If you have undergone a reduction, and you are planning to have kids, it is very difficult to look into that crystal ball to predict if and how your breasts will change. But common things are common. Post pregnancy changes to the breast can include volume loss and breast sagging and stretching of your areola.

Pregnancy does not treat all breasts the same way. Although some may stay more or less the same and others may become larger, the vast majority of breasts grow in size during pregnancy and after wards and stretch the skin envelope. When they go down in size, the larger skin envelope is associated with some breast sagging. This is not uniform but is fixable.

Breast reduction and pregnancy
I agree it is not possible to predict what the breasts will do if you have had a previous breast reduction and become pregnant. First of all, many patients want to know if they will be able to breast feed after having had a reduction. The best answer that we have from our literature is about 50%. It is actually (in my experience) probably better than that, but we let people know that we simply dont know whether one will be able to or not. I have had a number of patients that may have had breast reductions when they were very yound and then they had children and basically came back years later for a "second reduction". Typically it is more of a revision breast lift, but sometimes it can be a true second breast reduction. Some patients dont get much of any changes and continue to maintain their results. Effect of pregnancy on breast reduction is unpredictable.
Unless you are planning a pregnancy in the near future, you can go ahead with a breast reduction. The worst that can happen is that you need a revision through the same scars after you are done with pregnancies.
Pregnancy after Breast Reduction
Many women become pregnant following their breast reductions. Pregnancy almost always results in engourgement of the breasts. They usually decrease then in size following delivery assuming there is not breast feeding. You may well have some sagging following pregnancy, but this would be the case with or without previuously having undergone reduction.
Pregnancy after breast reduction, now what
Believe it or not, your breast will behave normally if you become pregnant after a breast reduction, and very many people do. The first sign you are pregnant may be a tender stiff feeling in your breast. Your breast will fill out during your pregnancy, the nipple may darken a bit, and after you deliver your breast will indeed become dense and firm and lactate. The success of breast feeding will depend on the pattern of your reduction and the remaining connection of ductal and glandular breast tissue. Very few breasts stay bigger after breast feeding and many will become a bit smaller, especially if you return to your before pregnancy weight.
