Breast Implant Sizing
Hello Bellachico,
I am sorry you want to be a D cup. Some surgeon could make that happen for you, probably with serious consequences. Get a tape measure and measure the distance from your nipple to the bottom of your breast while gently stretching the breast upward. You will need 9.5cm for a 400 cc implant. Cup size varies with chest wall circumference
If you were a size 30 or 32 bra then each 100 cc implant would be 1 size change. If you are a size 34 or 36 then each 200 cc implant would be 1 cup size change. Where you begin will determine where you will end based on your circumference. Implants placed retro-pectoral provide for better long term stability and less capsule contracture. The muscle compression will determine the projection more than high profile implant.

425 or 500 cc implants
I think your anatomy really helps predict what is best for you. If you choose a very large implant, it may be too larger for your frame and then you may have a double bubble type look of a too wide implant sitting behind a very narrow base breast. An exam would be critical.

Choosing cup sizes and implants in cc's
You should get an implant (saline-filled or gel-filled and low, medium, or high profile) that fits your breast dimensions and will give a predictable, natural looking result without stretching or distorting your tissues like a foot that's too big for a shoe or a body that's too big for a dress. High profile will be better
Due to your body frame, high profile will be a better choice for you, a surgeon with enough experience should be able to show you pictures of patients similar to you. At the end , the decision should be yours to make.

Best Implant Size for Athletic Frame
I agree with the doctors here who have concerns about an implant in the 400cc or greater range. You do not have a lot of skin, fat, or breast tissue to cover the implant so you may run the risk of appearing unnatural as your breast will be very rounded and essentially have the shape of the implant.

You should go with smaller implants
You have small amount of breast tissue and minimal skin over the lower pole of the breast. You should not get bigger than 300 cc implants. You will get thinning of the breast tissue and complications with larger implants
