Dear ElliotR,

  • Fat grafting is a great procedure because it does not leave any scars
  • It is usually injected from the midcheek area so you can address and access the entire lower eyelid
  • When you are ready, please visit a qualified fat transfer surgeon who can answer all of your questions and concerns

Fat grafting can be done from the nasolabial folds, but it can and should be approached from multiple different sites to get the optimum result. The beauty of this procedure is that the injection sites are accessed through a simple needle incision that does not leave a scar.

This means you don't really need to worry where the incision is made: nasolabial folds, lateral cheek, brow, etc

Your surgeon will place the incisions where ever it is most convenient to access the volume depleted region.

Fat grafting to the tear trough area can be achieved through the nasolabial fold, the eyelid, the crow's feet, the cheek, the earlobe, from within the mouth, etc.   There are many options here.  Find the plastic surgeon with ELITE credentials who performs facial fat grafting several times a week.  Kenneth Hughes, MD Los Angeles, CA

Entrance Site For Lower Eyelid Fat Injection Grafting

While fat injections to the lower eyelid can be done through the nasolabial fold area, that may not be the best approach to it. It is far easier and less traumatic to place it through the side of the lower eyelid or cheek area. The most important component of fat injections is to get good placement to the targeted areas not where the entrance point is. These tiny skin nicks heal imperceptibly, regardless of where they are placed.

Access points for fat transfer/injection

The access point for fat injection can be almost anywhere, including inside the mouth!  The key is where the fat is injected!  The access is usually done through small needle incisions, and best hidden in folds of facial lines.

Fat grafts

Yes, this is possible and really leaves no scar if a small instrument is used. Care is taken to place the grafts below the muscle at the level of the orbital rim in most cases. Good Luck!

Fat grafting access points

I usually place the access incision higher up in the cheek area when performing fat grafting to the lower eyelid region. The incision is very small and normally heals very well.
