What is the Most Effective Scar Removal Surgery for Deep Bacne?

I have had terrible acne on my back for around 10 years - i'm now 29 and I have severe scars as well as still having lots of cysts and spots. What are my options?

Back Acne Scars, Bacne?, how to treat them

It depends on what they look like. But you may need a combination of excisions or subcisions to get the scars to the same level as your normal skin. Then I would consider co2 laser resurfacing with a fractionated approach. A multilayered approach would work too.

Acne scars on the back are best treated with a combination of ablative and non ablative fractional laser treatments.

Acne scars on the back are best treated with a combination of ablative and non ablative fractional laser treatments unless the scars are keloids.  Keloids are thick, lumpy, painful or itchy scars that may also require injections of triamcinolone or 5FU.

Usually a combination of smaller procedures.

Subcision (literally having a NoKor needle placed into the skin repeatedly) can often significantly improve acne scars (see Dermatologic Surgery article by Murad Alam). Fillers such as Restylane can also help lift-up depressed scars. Fractional lasers, both ablative and non-ablative, can be helpful over several treatment sessions. There have also been reports, mostly out of Asia, on the use of high concentrated TCA at acne scars. Last, try to avoid scalpel surgery--as sometimes the scars can become even more prominent.
