and your particular case better defining the crease but keeping it in the same position would be of benefit. You could remove the skin between the lashes and the crease line conservatively while better defining the crease and extending it towards the medial canthus. I don't think he would need a full medial epicanthoplasty.
this can be done under local anesthesia and in general I would recommend an incision method. Again, do not aggressively seek an epicanthoplasty as this will more likely just cause scarring without really making her eyes look more open.
