I have large nostrils and a slightly wide alar base but when I use my fingers to ever so slightly push in my alars to simulate the results of an alarplasty, both my nostrils become obstructed and I have trouble breathing freely. How do doctors know beforehand if alarplasty will cause breathing issues?

An alarplasty can narrow the base of the nose and decrease nostril flare. Please submit a picture of the base of your nose so I can show you where the incisions would be made to achieve a natural result.
