I am 68 yrs old. Breasts are heavy, pendulous, uncomfortable, and skin irritations are ongoing. I would desire a smaller size, like a B cup. Is this done?

Unfortunately, no plastic surgeon is able to predict exactly what cup size you will be after breast reduction surgery. As you know, cup sizes vary depending on who makes the bra; this makes discussing cup size an inaccurate way of communicating your goals.
Also, every patient's chest width varies; if the patient has a wide chest wall it may not be possible to achieve a smaller width or cup size.

A DD breast can be reduced the a B cup
A large breast can be reduced to any size really although there are different techniques depending on where the patient starts and wants to end up. The issue is maintaining viability of the areola and nipple.

Well, it is possible that it can be done, but I would most likely not advise such an extreme change.
First of all, 68 (nothing personal, of course) is older than the average patient who undergoes this procedure. The issue is not necessarily your age (I'm assuming you're in generally good medical health...and not a smoker) but one of the blood flow to the central portions of your breasts.
