I have mild ptosis, about 1cm below the crease. What size/type implant is best without having to get a lift? My measurements are: sternal notch to nipple- 21cm; ribcage- 28in.; ribspan- 25.5cm; bwd- 12cm. I would like to be a D cup. I'm thinking Mentor smooth round hp with 400-425cc under the muscle. What are your opinions?

Minor amount of sag and desire for breast implant augmentation surgery

With your height and weight, I might prefer silicone in which case 425 is generally larger than I would recommend for your size but could achieve your results. In regards to ptosis, you could consider a crescent lift or vertical mastopexy either immediately or delayed

Size

Get what will make you happy. If those are the implants that you want and the volume you want then do it. Yes it is true for every body there is a volume that would best correspond n to you being proportional and symmetric but do you really want that? You really want proportion? From your question I do not think so. I think you want to be more voluptuous.  So go for it! :)

Rather than trying to decide via the internet which size implant to select for your problem. you should have a consultation with a board-certified plastic surgeon.  He/she can guide you through your options.  With a nipple 1 cm. below the inframammary fold, you mat need a small vertical mastopexy.
