Greetings. I was wondering why are chin implant are so small, the largest offering horizontal projection increase by only 10mm? How could such a tiny object make a difference in one's profile? I want to augment my chin since its deficiency is approximately 10-11 mm and I have the feeling that an implant won't suffice. However, I want to avoid osteotomy at this stage and custom implants are not available where I currently reside. Also, do implants cause bone erosion?

Chin implants do come in a variety of sizes. They can also be custom carved by the surgeon on occasion when there is mandibular asymmetry. Placing too large of an implant in the chin will make it look unnatural, and the skin envelope won't stretch far enough to accommodate the implant. The size of the implant is best chosen by the surgeon, not by the patient. Communication here is important as well between the surgeon and the patient. Bony erosion is something we notice when we remove a chin implant, however it has no consequences with the mandible.

As you know, there are a variety of styles and sizes of chin implants that give varying amounts of augmentation to the chin. In almost all cases, some style and size of implant normally exists that will be beneficial to the patient's appearance. If for some reason you need more projection than an implant can provide, it's possible that some strips of GoreTex could be placed between the implant and the bone to give further projection. Silicone chin implants have been shown to cause some bone erosion on some radiologic images, but this does not necessarily result in any physical change to the shape of the bone or any clinical problems.

Rather some chin implants may settle into the bone a few millimeters through a passive process of the bone adapting to pressure of the chin implant sitting on it from overlying soft tissue recoil.
