Causes of Reocurring and Spreading Melasma?

I have had melasma for 2y, it started as a small pygmented area on my cheek while i was on summer vacation. I have always been using sun protection (50SPF), even during winter. I had 3 IPL treatments and the spot would crust and flake, but the pygmentation would come back in less than a month and would be spreading. I have not been pregnant and have not taken any contaception. I used cream with glycolic and kojik acid for 4m and the patches faded, but came back much worse after i went vacation.
