Hi, i have a long midface (made up of a long nose) coupled with a prominent nasal spine, so prominent/low down that it pushes my lip down (pictured) and gives me collumellar show and makes my nose droop. Is a midface reduction a good option along with a anterior nasal spine reduction?
Answer: Rhinoplasty to lift the nasal tip and reduce the nasal spine The rhinoplasty procedure can reduce the columellar show which is caused from a prominent nasal spine. The tip can also be lifted as well with a tip rhinoplasty procedure and a columellar- plasty. It's also important to release the depressor septi ligament, which is pulling the tip down with animation. For many examples and more information, please see the link and the video below

Answer: Full rhinoplasty with resection of the tip muscle will give you what you desire. The muscle that goes from your nasal spine toward your tip pulls your tip down especially upon smiling. Your tip is long and droopy and therefore needs to be corrected to give you a more attractive nasal lip complex and nose. This will create a far more feminine and attractive look. See an experienced rhinoplasty surgeon for this.

Answer: Is midface reduction a good treatment for a prominent nasal spine + long midface? Following the advice from a surgeon on this or any other website who proposes to tell you what to do without examining you, physically feeling the tissue, assessing your desired outcome, taking a full medical history, and discussing the pros and cons of each operative procedure would not be in your best interest. I would suggest you find a plastic surgeon certified by the American Board of Plastic Surgery and ideally a member of the American Society for Aesthetic Plastic Surgery (ASAPS) that you trust and are comfortable with. You should discuss your concerns with that surgeon in person. Robert Singer, MD FACS La Jolla, California

Answer: Facial surgeries not indicated in your particular situation. There's not enough information in the photographs talk about very specific recommendations. The deformation of the upper lip could be treated with Botox. There may be a prominent nasal spine which is occasionally dealt with in a rhinoplasty. Unless I'm missing something any treatment more extensive than what I just discussed would not be indicated.
