A: Consider Candela Gentlelase for scars and large pores

Michael Law, MD

The Candela GentleYAG laser stimulates dermal collagen production and regenerates damaged skin cells. It produces new collagen in response to laser energy. Increased turnover of skin cells is also stimulated, helping to regenerate and replace damaged cells. In my experience, this laser is the ideal laser for... more

Laser may not be the solution in your case.  Generally, large pores, chicken pox scars, and acne scars have a low width to height ratio, thus making it difficult to treat with lasers alone.  Fraxel is not a cure for every scar.  It really depends on the depth of the scar and the quality of the... more

A: Best laser treatment depends on the nature, age, depth, etc. of the scars

W. Tracy Hankins, MD

No laser will be right for all scars.  Beware that the doctor or laser center is recommending only the laser that they just happen to have in their office.  Experienced laser surgeons should have a variety of lasers in their office, as well as be well versed in other techniques - subcision,... more

Hi mz, I have had much success in treating pore size, acne scars, and chicken pox scar skin surface irregularities with Fraxel re:pair laser.  Patients have had very impressive improvement in skin texture and scars after one treatment.  My patients are all told that they may require more than one... more

A: Fraxel Laser best laser treatment for skin resurfacing

Gregory Turowski, MD, PhD

Fraxel Restore is one of the best lasers for skin resurfacing. Fraxel laser works great for acne scars, pigmentation, fine lines, and wrinkles. Depending on the condition treated, Fraxel can be done in a series, or a single treatment. More aggressive treatments are necessary for acne scars, which mean more... more

A: Fraxel, Pearl, Laser Genesis all work for pores and scars

Jason Pozner, MD

All of the devices listed work somewhat but the most aggressive of the group, the Pearl, will most likely give the best results on texture, although the Fraxel may be best for deep acne scars. The best advice is to seek a laser practicioner with many devices so they can prescibe appropriate treatment for your... more

Unfortunately none of the treatments mentioned will completely correct the type of scars you describe.  They may make some improvement but the scars will still be visible. Also be prepared for some downtime after these treatments. There really isn't a great solution for facial scars from acne or chicken... more

VERY grateful for the candid and detailed answers all of you have posted! Many thanks. Even in the few consultations I've already had, none have offered the possibility of using a combination of laser procedures. So this is very helpful - thank you all.

Hi, I had a bad surgical scarring left after dermabrasion to treat my acne scars. In 9 years that follow, I've tried almost everything from Accutane, Obagi, Isolagen, Subcision, Derma-roller (micro-needling) to most recently, Fraxel. I was not aware that I had the older version of Fraxel 750 instead of 1500 for re:store (really disappointed!) After 4 months from the last Fraxel, of which I had 5 treatments spaced in 2 weeks apart, I realised that I look much worse than before Fraxel. It seemed that the scars have now become more obvious as though they are raised, hence showing more of the edges and the undulation or 'divots' which look worse under yellow lighting especially ice-pick scars. Can you explain why this is the case while seeing a slight improvement in the texture of my skin (smooth and a bit taut)? I am contemplating of going back to subcision and derma-rolling to thicken my skin so as to make the scars look less apparent, on top of some peeling. Is this the right route to take? If not, could you please kindly suggest the treatments I should or should not go for after Fraxel? Many thanks!

I have some of the same issues after fraxel White lines and now ulcers in the white areas that are leaving scars..I am reasearching ways to fix the scarring and ulcers...And the only thing I have come across that seems like it might offer hope is Valveta or structual fat grafting with the stem cells..washed or multiplied??. Because I don't need a lot of volume just the cells... I see you tried isolegen...How was it?.. You may want to look into it after a few months and see if the swelling goes down some more or the collegen builds......

The Pearl is amazing for Textural problems - fine wrinkles, spots  and moderate acne scars. The "down time" is minimal; 4 days, and the results are uniformly good. Genesis is great for non-acne (laceration and surgical) scars. Repeated treatments at high levels with local anesthesia work best. It essentially re-aligns the collagen and smooths the raised scar, and reduces redness. Fraxel is good for very deep acne scars, but healing is slow and results vary greatly patient to patient. I've seen patients who've had 5 treatments and still had scars. Good laser docs are well prepared: 1) They have several lasers for multiple and even combined treatment options, and 2) They perform the procedures themselves. Don't let "laser techs" wield these machines on your face!

Laser genesis will work great for pores but not really that good for scars. Bear in mind that acne scars in particular are hard to treat. The Pearl laser has both a laser peel and ablative fractional resurfacing capability. That combination done together will give better results as compared to the Fraxel Re:pair which is an ablative resurfacing laser but does not have a laser peel. Note however, that the fractional Pearl which is quite new will be available in much fewer practices than the original Pearl laser peel. The peel alone would give inferioe results to the Fraxel Re:pair. there are other lasers that will also give excellent results.

My board certifiend plastic surgeon has recommended the a Fraxel procedure which he said is a one time treatment. My skin is fair and my pigmentation on my face and neck is very even (beautiful) but I am bothered by crepe like wrinkles and sagging around my mouth. I have pigmentation problems on my upper chest from incidental sundamage. He is going to treat my face neck and chest. He recommended an obagi product with hydroquinone prior to the treatment. He is even delaying treatment because he wants me to have used it for at least a month . I have been advised on another forum that hydroquinone before laser treatment is "a big no no". (direct quote). These opinions come from women in the Medical Spa business. Why are their opinions so different from my doctor? And do you think that this is the right treatment for me?

Kath, I think you should trust the doc who is treating you. He is the one most familiar with your case.
