My PS asked me a very good question. Would I rather reduce size OR be perkier? I said I really wouldn't be happy with just one! Right now my size is around 30/32F. I consider my breasts deflated but my PS said my aerolas are down just about 1cm from my crease. Also, he said breast placement on my chest is slightly low (agreed). I want to be SMALLER & PERKY. I imagine a B cup. PS recommended a lift and a slight reduction, and more discussion. I just wonder, is what I want even feasible?
Answer: Perky B after reduction? Thank you for your questions and photos. The answer to your question is yes. Yes, you can have a reduction/lift and get perkier breasts that are an approximate B-cup. Without an exam to determine stretch, tissue quality, amount of dense breast tissue, etc. it is impossible for me to determine and advise exactly what you could expect. However, with breasts similar in appearance/size to you, my patients have received very good, perky results with a central-mound/Wise pattern (keyhole/anchor) type lift. After your reduction/lift there will never be an abundance of breast tissue in the upper portion of your chest, but I find that the majority of my patients are very happy with their breast shape and extremely pleased to not have the extra weight pulling on their neck. If, after you have completely healed from your reduction/lift, you have a desire for more upper pole fullness, a small implant can be added for that purpose. Please review the photos below to see some examples of similar patients/breast reduction results. I would advise you to seek consultation with several board certified plastic surgeons to find the one that you feel most comfortable with. Best wishes with your procedure!

In regards to breast size desired, it will be important for you to communicate your goals carefully. In my practice the use of goal pictures are helpful in this regard; a discussion of cup size can be confusing and imprecise. Therefore, I would not suggest that you communicate your goals and/or base your satisfaction with the outcome of surgery on achieving a specific cup size. I also find that the use of pictures is more helpful than the words “natural” or "proportionate” etc., which can mean different things to different people. Many of my patients choose to have enough breast tissue removed to help alleviate symptoms while retaining enough breast tissue to remain proportionate to the remainder of bare torso. There are techniques to help with this goal. Using an inferior pedicle sutured to your muscle can help with superior fullness or the use of a Seri graft which helps support the tissue and does not let if sag as much. Speak to you PS and listen to what he or she recommends. They have to work with what is possible and not what is wanted.
