I want a thinner & defined front profile.Do you think it would be possible to achieve this result under local anesthesia?(photo)

After having Rhinoplasty i noticed that my doctor did not alter the base and i still have a bulbous nose. Its been 1 year and 2 months since i got my primary rhinoplasty procedure. I wanted a thinner and defined front profile. I have attached my current front profile and then used a nose simulator to give you an idea of how i want my nose to look like. I only adusted the sides of my nose and slightly the tip. Do you think it would be possible to achieve this result under local anethestic?
