1.Will this scar heal well?HOw long?had an injection to'flatten'the raised scar today,now using kelo-cote 3times/day.

2.Can undergo a second excision?Will it cause larger hypertrophic scar?

3.Still possible to achieve thin fine line after this first excised scar?

4.Why this occur?scar lacks moisture?fever/cold affect scar?

Keloid scars respond to 5 fluorouracil and triamcinolone injections. These scars are due to an overproduction of collagen and the injected medicines will reduce the excess collagen.

Bleomycin is an excellent treatment for hypertrophic scars as is 5FU

By far the best treatment results I have seen with scars such as this is Vbeam laser followed by intralesional 5FU and steroid injections or bleomycin injections. I would recommend continuing with your excision and have the incision scar treated with the laser monthly starting 2 weeks after suture removal. If it again begins to thicken up, immediately add the injections to the laser treatments. Best to you.

Hypertrophic facial scar

I would be interested to know if this wound healed up well initially, or if there was any delayed healing or infection.  This looks like a hypertrophic scar, which is usually self-limiting, although can take 18 months to settle.  I would advise massage and moisturise in the first instance, moving on to silicone ointment and possibly steroid injection if it doesn't settle. Keloid after excision of birthmark

If a keloid formed after a small excision of birthmark, chances are good that a larger keloid will form after a larger excision. It is not possible to achieve a thin line in this scar.  It can be flattened with Kenalog injections but the width will be what it is now. Laser treatments with pulse dye laser can also shrink the redness and the height of the scar. Thanks for the photo. Looks like a hypertrophic scar has formed. I would finish the serial excision of the pigmented nevus in a few more months. In that time I recommend silicone sheeting.
