Is It Advisable to Have a Nipple (Not Areola) Reduction and BA at the Same Time?

I have very large protruding nipples and a smallish areola. After breastfeeding, they started to droop and now look worse. I would like to have a breast augmentation, preferably with an areola incision, could I also have my nipples reduced in length and width at the same time? Or would it cause too much stress or trauma on the area? I don't need a breast lift (if that matters) and according to the PS I saw during my first consult, my areola area is large enough for an areola incision. Thank you.

Nipple Reduction with Breast Augmentation

Reducing a nipple that is too long or too wide (or both) can be done as a matter of routine at the same time as breast augmentation.

Nipple reduction is possible

Nipple reduction is certainly possible at the same time as a breast augmentation, regardless of augmentation incision. The important fact to keep in mind is that lactation will be interrupted due to the inherent scarring. If you are planning on breastfeeding children in the future, you would want to delay this procedure. If you are terribly concerned with performing both procedures at the same time, the nipple reduction is simple to perform in the office at a later date under local anesthesia. more

This is a reasonable combination though there are some theoretical concerns about increasing the bacterial exposure to the implant and possibly increasing the risk of capsular contracture.

Nipple reduction and breast implant augmentation

This combination of procedures can easily be completed at the same operative session. Some surgeons have concerns about biofilm formatiion and transmission of bacteria through the lactiferous ducts.

Nipple reduction at same time as breast augmentation? Yes

I have performed exaclty this combination of procedures, and it works well.  The nipple reduction really removes skin around the base of the nipple to reduce its height, so that's not terribly detrimental to its blood supply.  If your nipples also require a change in diameter, then an addiitonal incision needs to be made to essentially remove a "pie slice" of the nipple and make it narrower.  I think the key to any blood supply problems (which could kill all... more

Yes to Nipple Reduction

Nipple reduction is an easy, quick procedure to correct nipple hypertrophy. It can be performed at the same time of any breast procedure or on it's own as a local anesthetic in an office setting. I am surprised we don't perform more of this procedure. With Warm Personal Regards, David R Finkle, MD

Dealing with large areolas during a breast augmentation

An areolar reduction can be done at the same time of the breast augmentation. I will say though that when we are doing surgery, we desire to have the least amount of tension on the incision in order to get the best quality scar. Increased tension will potentially cause the scars to heal wider or thicker. Nipple reduction can certainly be done at the same time as breast augmentation. There are a number of techniques to reduce nipple size. Some may permanently interrupt the milk ducts or nerve fibers. Discuss the options with an experienced plastic surgeon.

Yes, this is straightforward, and is routinely combined with a breast augmentation.  I use a technique that telescopes the nipple to a shorter length, preserving the internal structures as much as possible.

Nipple reduction and periareolar incisions in breast augmentation

Nipple reduction is a rather simple procedure that can be done with local anesthesia and heals very easily and quickly like an incision inside the mouth even if it wasn't sutured. Areola Reduction

Areolar reduction is safe and easily performed at the time augmentation.  I would not do them separately as there is no reason to.  There is a slightly increased risk of capsular contracture when an implant is inserted via a periareolar incision as compared to an inframmary incision or transaxillary incision.  But the periareoar incision is used quite often and should lead to a nearly invisible scar.  Good luck!!

Nipple Reductiona and Breast Augmentation

Hello, There should be no concern having a nipple reduction simultaneously with a breast augementation.  If a breast lift is also being performed however, than a nipple reduction should be deferred for another time.  The reasoning behind these decisions is based on preserving circulation to the nipple area.  In the situation of breast augmentation alone, there is sufficient blood supply going to the nipple to allow it to heal well.

Great questions! Yes, reducing the size (and lifting) your nipples at the time of a Breast Augmentation is feasible. Reduction of the nipple at the time of a periareolar breast augmentation is very reasonable. However, with “smallish” areolae, if they are too small, you may have to choose between periareolar augmentation with saline implants and another approach for silicone implants. I would consider doing them boht at the same time , except for the circumstance of using a periareola incision for placement of a circumareola approach in an areola lift.  In this instance I would then stage it.

You can certainly proceed with a both a breast augmentation through a periareolar incision and a nipple reduction procedure at the same time. The periareolar incision, since it does not require any undermining of the areola itself will not impinge on the blood supply to the nipple. The nipple reduction procedure is very simple and straight forward and would be certainly advisable to perform at the time of your augment (once the augment is completed and the wound is sealed up). more

The nipple does come in all sizes and shapes, and most will be concerned about the excessive size of the areola. Some however will have a very large, or a very elongated nipple which can be caused by breast feeding. There is little risk to the breast implant if reduction of the nipple is completed after the implant is in place and the pocket closed. Nillple Reduction & Augmentation Can be Performed Together

Although there will be some interruption of blood supply to your nipple with these two proximate incisions, I would not expect a problem unless you are a smoker.  If you do smoke, I definitely would stage the two procedures.

Nipple reduction with breast augmentation.

I have done this procedure successfully. There are theoretical concerns about introduction of bacteria from the ducts. I suggest to have the augmentation done first with closing of the incision and then procede to the nipple reduction. Most of the time this involves removing of skin only and not violating the ducts.
