Advice for Clearing Up Red Patches on Face

Have developed a couple red "patches" between my mouth and chin. Possibly started as post-acne blemishes? Sometimes more prominent than others. Realized these have been around for a couple years now so just started getting more aggressive. I started using an Aveno night cream or a Eucerin redness reducing night cream, switched makeup I was using to even skin tone thinking it might be a reaction. Too early to gauge effectiveness of these. Any help appreciated.
