In addition to the notes below occasionally insurance companies will cover a breast lift for breast asymmetry. If there is a big difference in the shape of your breasts correcting that may be a covered benefit.

If both breasts are the same size (approximately) and you are happy with the overall size of the breasts but just want them lifted - that will be likely harder to get covered.

The other thing to remember about insurance is sometimes submitting appeals can be an effective way to change an initial denial into a yes.
