The more breast tissue you have the less you will feel the implant.  Saline tend to be more palpable and can feel like water balloons if you are very thin, expecially at the lower border of the breast.  Silicone gel filled implants tend to feel more like natural breasts as compared to the saline filled ones.

In the good results, breast implants feel like normal breasts. There can be two complications that can make breast implants feel less natural.
            About 5% of patients will develop capsular contracture which causes some firmness around the implant. In this case, the implants do not feel hard, but they feel firmer than typical breast tissue. I tell my patients that a breast with capsular contracture may feel more like your thigh, which is usually firmer than the typical breast. 
            With saline implants, there is some risk of noticeable rippling. Rippling means that when you feel the breast - especially on the outside - you can feel a waviness under the skin which suggests the palpable contour of the implant. When rippling does occur, it is typically not visible, but it can be palpable when you touch the breasts. One of the reasons that we usually recommend silicone implants is that rippling is much less likely to be a problem with silicone implants that it is with saline implants.
