Would Accutane Work for the 3rd Time?

I have taken 2 courses of Accutane for the past 5 years. The first course for 4 months, then acne returned then the second for 6 months but the acne returned again. Both with 40mg/day dosage.

I tried another doctor who tried tetracyclines, and when they all didn't work, he tried hormonal pills like Diane, they also didn't work.

He gave up, but was against 3rd course of Accutane. I consulted another doctor and he said only Accutane would work. He suggested a 1mg/kg dosage. I'm not sure which doctor to trust? Will a 3rd time work? Will it have long term side effects?
