I would advise seeing your dermatologist to review the treatments for your painful acne. There likely is a hormonal element, so blood work and/or pelvic ultrasound may be warranted. Likely you'd require oral therapy with an oral contraceptive, and/or oral antibiotics or isotretinoin, although laser or photodynamic therapy may be fairly helpful along with medicated creams. ~ Dr. Benjamin Barankin, Toronto Dermatology Centre.
Painful recurrent pimples probably require combination treatments that include aggressive topical prescriptions and oral antibiotics +/- oral contraceptives. Non prescription over the counter remedies and options will probably not be sufficient. I would suggest an evaluation by a dermatologist for possible prescription therapy.
