W. Tracy Hankins, MD answers: Redo Canthopexy to correct eye shape?

I had a Blepharoplasty and a midface lift. My eyeshape was altered in such a way that the lower outer half slopes downwards. I had a canthopexy and tarsal strip procedure to correct this 2 months ago. It didn't do any help.

What do you think would be the best way to go now? Should I have the canthopexy redone? Or would a hard palate graft be the way to getting more height in my lower eyelids? What about from behind the ear? Is there a problem with either of these grafts? Is there a more permanant fixture?

It sounds like you have a fairly complicated problem.  You shouls eek out a surgeron who has a great deal of experience in eyelid surgery and you should come up with a plan that you are comfortable with.  I personally prefer a capsulopalpebral fascia release and an interposition graft using either autologous dermis or alloderm along woth a strong canthal support procedure.
