William Bruno, MD answers: How to choose breast implant size?

I'm worried that if I get breast implants my A cups with too large implants will give me a top heavy look (my friend calls it the stripper look!).  Is there a rule of thumb or tips for choosing breast augmentation sizes?

Breast implant size is determined by both your plastic surgeon (board certified) and you the patient. The implant should conform to your particular anatomy and body type. The implant can only be as large as your tissues/skin will accomodate. Often times patients want large implants that don't necessarily correspond to their body type or are too large for the skin envelope or tissues. 

There are certain measurements your surgeon will take such as breast width, which help guide the implant selection process. Once you have agreed upon a range of implant sizes based on your anatomy, the surgeon should use a sizer or a temporary implant that is inserted during the actual surgery to assess the elasticity of your tissues. This sizer helps to guide the ultimate size selection and helps to accurately shape and develop the pocket where the implant will reside.

Please consult with one or more local plastic surgeons to discuss which size is best for you. You may also try on some of the different size implants before hand in the office to get a rough idea of how the implants will look when completed.

A: Measure, then choose the right breast implant size with your surgeon

The patient's natural breasts have a certain diameter. In our practice we believe the implants should not be larger in diameter than the patient's natural breasts. Since the implant types and shapes are well known, you can figure out the maximal size, and decide from there.

Remember, the larger the implant, the more destruction of the muscle attaching to the sternum must be to fit in the large implant, and the more the implant will stick out into the armpit. The surest way to have surgery after surgery is to pick very large implants.

Breast implants are manufactured in different sizes, base diameters, and heights.  There are low profile, medium to medium high, and high profile implants.

Choosing size is best determined by assesing your individual body aesthetic and breast diameter.  There are no wrong answers, but body harmony is ideal.

Tips for choosing an implant size:

  • Wear a supportive, non padded bra to the consultation.
  • Bring a white or black tight blouse with minimal designs or fluff to assess your body dimensions and proportions.
  • Take the time to try different size and shapes of implants.  Insist on trying the medium, medium high, and high profiles before deciding.
