I have not even seen your photos, and I can tell that you will need:
1) removal of your current implants;
2) replacement with smaller implants; and
3) Breast lifting, most likely a full lift, but depends on your degree of sagging.

The fact that you want to go smaller is good since larger implants will always cause more problems in the long run than smaller implants. Smaller implants, relative to your natural breast tissue, will always give you a better chance for a more natural breast compared to larger implants. In addition, the fact that you feel your breasts are sagging to your waist indicate that you will need some sort of a breast lift or mastopexy. You have effectively made your own diagnosis and recommended your own solution to yourself.
You will need to consult in person with a plastic surgeon to determine what type and size implants would be reasonable for you to achieve your size goal, how to perform the breast lift as there are a myriad of ways to do the lift, and what the risks there are to performing the implants exchange and the lift simultaneously as there may be, but not always, an increased risk of complications when performing the two operations together.
Breast implants may exacerbate aging of the breasts when combined with pregnancy, weight gain, and loss of skin elasticity. In this case, it seems that the droopiness of the breasts can be improved with a breast lift procedure that will reposition the nipple and areola and remove excess skin.
