Saline breast implants can look and feel very natural. It LARGELY depends on the patient and how much breast tissue she is starting with to cover the implants. Standard of care would be to place the implants under the muscle. Speaking to your surgeon about your expectations is very important. Taking in pictures of breast sizes that you like and also sizes that are too small and/or too large is also very helpful for your surgeon to understand what you are trying to achieve.
