The box tip or ball tipped nose a fairly common problem due to large cartilage that is shaped like a ram's horn. This runs along the upper edge of your nostril and extends down between the nostrils. I usually approach this by making an incision in the skin between the nostrils (the collumela) and lift the skin and subcutaneous tissue off of the cartilage . I then trim it and suture it together if necessary. If the overlying skin is very thick, change in the shape will not be as noticeable as when the overlying skin is thin. In addition ,there are some conditions where the tip of the nose is large due to congested blood vessels. This is the "WC Fields" type of nose. This is a condition called rhinophyma and is treated with carbon dioxide laser.
