If you have enough breast tissue to allow the surgeon to sculpt an aesthetic conical mound without needing the extra volume of an implant, you can have a mastopexy with no implants. This is what I personally do in about 15% of my mastopexies. The only other issue you need to accept is that you won't have superior pole fullness without an implant.
So, much depends on your own starting point and your expectations. You can always do the lift and if you want more volume after 4-6 months of healing, an implant can simply be inserted through the scar you already have.
