According to the picture the patient has brow ptosis and a browlift would seem to be in order. I would probably hold off on doing blepharoplasty based on the amount of excess skin present once the brows are lifted and resuspended. You can always perform blepharoplasty as a secondary procedure down the road if needed. During the browlift, the corrugator muscle complex can be softened along with the frontalis, creating forehead and frown lines.
